Palliative Care

Palliative Care is a holistic approach of providing comfort, pain management and symptom control of the patient’s physical symptoms. The goal is to prevent and ease suffering. Palliative Care is appropriate at any time in a serious or life threatening illness and is not dependent on prognosis. Palliative Care can be provided at the same time as curative and life-prolonging treatment. Palliative Care is a comforting step for a patient who is facing mortality and wants some support in meeting curative or longevity goals.

Palliative Care is not the same as hospice care. Palliative Care is intended to empower patients while they manage the course of any chronic illness, optimizing their quality of life by anticipating, preventing and treating their suffering. Palliative Care supports the patient’s and family’s goals for curative treatment or longevity.

Palliative Home Care includes:

  • Pain and symptom management for supportive care is emphasized.  More aggressive therapy may be provided/ordered by the primary physician. Patients may go back to the hospital if desired.
  • Durable Medical Equipment, Oxygen, and Prescriptions if provided under the patient’s health plan benefit.
  • 24-hour on call support for questions, telephone support, and visits as needed.
    Education about medications, disease process and symptoms.
  • Emotional and spiritual support for patients and family members.
  • Connecting patients and families to additional resources as needed.
  • Communication with the patient’s physician and other health care providers to coordinate care.
  • Assistance with transitions to other forms of care, such as to hospice if needed, or back to their own daily routine if they no longer need services.
  • Bereavement support through support groups offered by HomeCare of Mid-Missouri.

HomeCare of Mid-Missouri’s Palliative Home Care team includes physicians, registered and licensed practical nurses, home health aides, physical/occupational/speech therapists, social worker, dietician and chaplains. All services are provided under the direction of the patient’s physician.
